This Student Assessment Booklet includes all your tasks for assessment of CHCHCS001 Provide home and community support services.

ABOUT YOUR ASSESSMENTS

This unit requires that you complete 6 assessment tasks. You are required to complete all tasks to demonstrate competency in this unit.

  1. What is the purpose of the Community Care Common Standards?
The concept of community care is to provide the necessary help to those people who don’t have anyone near for their care and if they have they live far away for their job purpose and can provide the financial help and could hire someone in their place to help and support their family in term of whatever they need.

As we all know about the old shelter homes in which people can live their the last stage of life in total care similarly community care offers the same facility at the house of the person (Elliott, et al., 202)

  1. What are the three Community Care Common Standards?
Standard 1: The person who is going to visit the place of the client must possess the necessary knowledge that has been required.
Standard 2: Must be suitably fit as described by the client.
Standard 3: Must have detailed knowledge about the working so that he can perform his duty as per need.

Briefly explain the purpose of the following community care programs. For each program list the services available.

Research current directions that the government is taking in relation to community based care.

What are four reforms that are planned?

(You may like to access The Coalition’s Policy for Healthy Life, Better Ageing (September 2013).)

They are taking many steps in the direction of helping the aged people in Australia.

The four reforms that have been planned by them to do are as follows:

1 they have made a 5-year agreement program for the older person for their better days which are they having in their life.

2 cut red tape

3 the major and most important that has been included in the plan is the investigation at the different workplaces or aged care and surprise inspection kind of form to check the authenticity of the place.

4 and the second topmost work that has been provided to them is investing in the research of dementia (Hapipa, & Bhuvaneswari, 2020).

  1. Briefly explain the term ‘duty of care’.
Duty of care is directly meant to help the person who needs it. At the care center no matter what the person is doing and gets up in a situation that is not good for him or is his or her caretaker it becomes the responsibility of the care taker to take good care of the person in every possible manner and do his job done well. Basically this all included in term of duty of care (Ireton, 2021).
Marco is visiting Kim in his home. Kim has mental health, alcohol and drug issues. When Marco gets to Kim’s house, he sees that she is clearly under the influence of drugs. Marco is unable to raise Kim, who has passed out on the lounge. 

Marco is annoyed as he told her he would be coming today. Marco decides to leave Kim to sleep it off and come back later in the day to complete his visit.

  1. Has Marco demonstrated his duty of care to Kim? Explain your answer.
No he has not done his duty properly as he has shown the person passed out on the floor in such a case the first thing that he must do is to help her out and put her on bad and check for any injury if there is and after that in the morning he could advise and guide him properly (Leff,  et la., 2015).

You are a community services worker in Adelaide. There is a new client, Alf, who requires support in his home while he recovers from a stroke. 

It turns out that Alf lives in squalor and is a hoarder.

What are five controls that are suggested to work safely in this situation?

Control 1: A safety plan designed for the patient as per his disabilities can be used in the time of need.
Control 2: The person who has the right knowledge must be sent to take good care of the client
Control 3: The person who has attended the same category of people before could have been appointed as he has the experience based on the past which will make him a suitable candidate for the job as in the time of need he can assist in most suitable manner.
Control 4: The medication must be checked regularly if the does have been changed or running out of medication it must be refilled at the right time to avoid any gap in medication.
Control 5: Getting a proper record of the client of his daily activity and eating habits will be helpful in times of need when a doctor can provide more accurate treatment if something goes wrong.

  1. List the legislation (title and date) that regulates privacy in your state or territory. Briefly outline the purpose of this legislation.
Legislation: The privacy act 1988
Purpose: It serves the handling of personal information of individuals, and it includes the collection of personal information, use, or even the storage and prohibit to disclose any information to anyone in the public or private sector.
  1. What are the key points of privacy legislation in relation to home and community care?
The privacy point in regarding to home and community care could be as they are going to work with the elderly aged people who aren’t capable of doing things by themselves in this process they are going to get some of the really sensitive data because of work-related but that doesn’t give you an option to disclose it to anyone only work related data is excepted (Marikyan, et al., 2919).

  1. What are the major consequences of breach of confidentiality?
It could affect the person whose data has been disclosed openly and the person who did it will go to face some the problems legally as being charged with doing such a thing (San Miguel, et al 2016).
  1. To whom should you refer breaches of confidence or other standards?
To the senior in the department who is Handel such kind of work as his duty. And if it is not doing his job correctly than go to the upper head in the centre to complain.

  1. Write down the responsibilities of employers and employees in relation to WHS in a home and community services environment.
Employers: being an employer of a care home it becomes a very important responsibility to take proper information on the client to avoid any kind of wrong care (Smales, et al 2022).
Employees: the responsibility that must every employee perform at home care is doing their job correctly as people who are not able to do so by themselves and have nobody in their family to do the job they have appointed someone and is being expected from them that must perform their duty right (Stocker, et al., 2018).

  1. Provide two examples of how you would meet work health and safety requirements in the following areas.
Maintaining personal hygiene
Example 1: regularly performing each day’s task at the right time and with complete care.
Example 2: fulfilling the desired thing if they are running out of stock so that it wont creates a scenario in which the client has to feel that it is being not cared for well.
Wearing appropriate personal protective equipment (PPE)
Example 1: using each described ppe as per the requirement and avoiding any sort of contamination
Example 2: checking for the breach in PPE and maintaining the hygienic environment for self and others.
Ensuring safe manual handling
Example 1: having gloves, proper kit if requires, and maintaining the necessary guidelines which are being provided in the area.
Example 2: taking necessary precautions to avoid unwanted threat which could lead to some other kind of problems or issues.

Question 8

Consider the following situations. Identify:

  • the hazards and associated risks (at least two for each item)
  • possible control measures the employer should put in place to minimise the risk of injury or illness
  • action you should take in each situation, including any reporting requirements that need to be made.

  1. A person-centred support approach can contribute to the empowerment of people living in the community. What is the advantage of this?
It will let the person feel that he is not isolated from the surroundings and has equal values as anyone in his surrounding.  In this style or approach person is supported in a way in which they can take their own care and also in this style they are treated as a person first. The major thing about this approach is to focus more on what are the person’s capabilities are and what they can do and even what their disabilities are.

Some basic principles on it are providing care, support, and treatment. And also to offer personalized care, support, treatment, etc.

  1. In one to two sentences explain why older people, or people with disabilities, are commonly disempowered.
Because their body isn’t capable of doing any more physical work and they have to depend on the other for their day-to-day life activity which leads them to a disadvantage and it results in disempowered older people.

  1. Read the following scenario and answer the questions:
Alberto has had a stroke. 

Although his wife, Michaela has been able to provide love and support for Alberto, there are some things they just couldn’t manage.

Alberto and his case manager, John, have talked about his needs, with Michaela being involved in the discussion. 

They are adamant about what Michaela can do for Alberto and the types of support they need. They ask for help with Alberto’s showering, dressing and transport as Michaela does not drive.

Alberto also wants to attend physiotherapy to regain his strength.

After a few months, Alberto is very happy with his progress. He is now quite capable of showering and dressing himself and has just been given permission from his doctor to drive again.

John meets with Alberto and Michaela to discuss changes to his care plan. It is agreed to cease the personal care and transport services, but in place of those John will receive some assistance with gardening and general maintenance, as he still finds balance and heavy work difficult.

  • In what way does this scenario represent consumer-directed care?
As each and every client has different needs and we can clearly see that arrangement has been made as per the requirement of the client which shows how flexibile the care home service provides. And it is based on the consumer directed care.

  • How do you think this approach has helped Alberto to recover from his stroke?
He gets what he has wished for as no one is more capable of doing but after with some of the help provided by the people let him do some of the daily work and he can cope with his life which is actually helpful to him. We can clearly see at the end of the scenario that he is happy which is an indication that the approach is good for him and going as per expected.

Question 10

Elsie is 80 years old. She lives on her own in an old country property five kilometres from town. Her nearest neighbour lives over 500 metres away and is often away from home. Elsie does not have a mobile phone.  The house needs repair aarrangements leaking, but Elsie does not have the money for this. 
  1. What action could you take to make sure Elsie is safe, balancing her wish to be independent and be able to take risks?
After knowing the issue the most appropriate answer is to help in at the level of the community with some sort of donation to get her roof repaired and get a person to check on her daily need to avoid any kind of mishappening to her.

  1. List four things that could be done to reduce risks to Elsie’s safety and security.
Action 1: Timely checking

Question 11

Gianni lives alone at home. His eyesight is deteriorating rapidly, however he is fiercely determined to stay in his own home until he dies. 

He walks to the shops each day at 10.00am to do his shopping, but he is finding it harder to do with his bad eyesight. 

The town where he lives is not the same as it was when he moved there fifty years ago. In those days it was a nice country town – now it is a growth area in the outer suburbs of Melbourne. There is a large problem in the area with high rates of unemployment and large rates of methamphetamine use.

  1. What safety and security risks does Gianni face?
She can get into an accident if she is not aware of her surrounding. And because of her poor eyesight, she can cause trouble to others too and it will also an indirect reason for  the situation in which she doesn’t want to get into.
  1. List four things that can be done to reduce risks to Giannis’s safety and security.
Action 1: Providing the necessary care for her
Action 2: Fulfilling her basic needs
Action 3: Consulting to doctor for her eye operation.
Action 4: Letting him a third person help in daily life.

  1. What procedures can be put in place to ensure fire safety in a client’s home?
Having sand bucket for the electrical fire and fire extinguishers also which are working in condition and pipe  roles for water to transfer from tap to required place.

  1. Research the legal requirements in your state or territory for smoke alarms to be installed in homes. Briefly explain the requirements.
In places where many people visit as shops and other places such as the restaurant their risk of getting fire is hired that’s why the government has issued notice for such places and nobody can operate without having fire security. A S 1670 automatic fire detection and alarm system is must.
  1. What is the name of the Act or Regulations that cover requirements for smoke alarms in your state or territory?
AS1851 FIRE SAFETY act.

Question 13

Explain the procedures you must follow when entering a client’s home to ensure them of your identity.

The basic procedure that must be followed as an employee of the care home service is that whenever you are visiting the house of a client you must have all the required document, properly introduce them to the client, and use the appropriate tone of voice to let them feel comfortable with you and provide them intel about the procedure and things which they haven’t ever experience before.

Bunty has cancer.

She has undergone one program of chemotherapy and has been receiving HACC services due to her declining health needs. Bunty was very sick during the treatment, but her health gradually improved.

Last week Bunty was distressed to find out that her cancer is back. Her oncologist has strongly recommended a further chemotherapy program. If she does not have treatment she is likely to die within a month.

Bunty has decided not to have the second lot of chemotherapy – instead she wants to die at home. Her family are angry that she is giving in to the disease – especially as the oncologist has so strongly recommended further treatment.

  1. Should Bunty do what her family and oncologist want her to do? Explain your answer.
Yes as every person is thinking in favor of her she must also listen to the advice of their family and doctor so that everyone will be happy and this is also the right decision as giving up on something is not a valuable choice.
The family approach you and ask if you can talk some sense into Bunty.

They ask you for legal advice about whether Bunty can be forced to take the treatment. They also ask you lots of technical questions about Bunty’s cancer and how treatment will work.

Bunty has given her consent for you to share information with her family, but you don’t really feel this is within the role of your job.

  1. What should you do in this situation?
As the disease has created a negative atmosphere in the families life but at the end of the day they all want happiness for each other even the bunty as she is feeling that she is becoming a burden on his family so she has decided to give up on her life and helping them is seems a right option in this scene as being mediatory both parties will convey there feeling and will listen also so this will be a right choice to perform.

What is meant by ‘emotional abuse’? Provide examples of emotional abuse that older people and people with disabilities are commonly subjected to.

Emotional abuse refers to a way in which controlling another person by emotional shame or blame. And things related to this will define as emotional abuse.

For example, if a son and his wife taunting on their parent because of their disabilities and provoking them to leave home is a good example for emotional abuse.

Case Study 1: Bernie and Glenys
Bernie and Glenys have been married for 47 years. Glenys has been diagnosed with dementia and requires constant care. Bernie has been caring for Glenys at home and is committed to her care. 

Now Bernie has found out that he has terminal cancer. He will probably only have about six months to live.  Bernie is distraught and worried about what will happen to Glenys after he dies.

One day while you are cleaning their bedroom, you find a stash of medications hidden behind the cupboard.

When you ask Bernie about them he looks very guilty. He says that they are his ‘insurance’ to make sure that neither he nor Glenys will suffer. 

He also says that you should not say anything to Glenys or anyone else about them – he couldn’t bear for Glenys to be lonely when he dies.

You look at the medications and see that these are very strong. You realise that Bernie is considering ending his own and Glenys’s lives when he feels they can no longer cope. 

  1. You sympathise with Bernie’s situation, but what should you do in this ethical dilemma?
After reading the scenario it is clear that they love each other very much and he made doing things that are unappropriated but his intention is not wrong at all he does care about her wife and he wouldn’t take such action if he hasn’t been diagnosed with cancer and he is having fear  what could happen to His wife after his death so he decided to and his and her wife’s life together to solve the issue the best action in this scenario is to hear them what they have to say about it and then provide them some psychological help because they are having too much in their plate to deal with and psychological help will definitely going to provide them a direction to move on
  1. Identify two possible solutions that will assist Bernie to address his concerns and be comfortable to take different ‘end-of-life’ action.
 We already have talked about please in the previous question that they must visit create psychological doctor who could handle the scenario in a most appropriate manner and could provide the necessary information and direction for them to follow this could be my first advice in this scenario them my second solution for the question just to have trust on each other and have complete transparency between them together they can do anything a personal loan taking all the burden it will end up in a messier situation which indirectly another person has to pay help from family and friends Would be a better option

  1. How would you demonstrate respect and sensitivity to Bernie – imagine that your views are very different from his in this situation.
He was doing everything correctly and as per the need for his wife but unexpected circumstances which have been occurred with him demotivated him toward the solution which wasn’t the appropriate majority of people hi I don’t have many wrong intentions but the idea which he has been come up with is wrong we also know about that what he is going to do is very wrong but he can’t let her suffer alone that smart his mentality mining situation like this peoples generally get 10 stands and have been implementing wrong decision which could damage lives of the person in the family.
The workplace grapevine quickly becomes active. One of your colleagues comes rushing up to you, ‘Oh my goodness, I can’t believe that Bernie was going to murder his wife!’ she exclaims. ‘What did he say to you? Is he going to be arrested? Poor old Glenys – he kept on saying he wanted to care for her at home but all this time he just wanted to get rid of her.’  
  1. Explain how you would respond while still maintaining confidentiality.
Doing each and everything as per the right procedure and following all necessary guidelines basically means not doing anything which goes beyond the legal limits.

Before taking any kind of step getting the whole information about the topic in-depth is necessary to discuss it with the team so that they can maintain the secrecy of the matter while solving it.

And if they find out something which requires legal help then before proceeding with anything they must discuss it and only do things which is possible to reverse because in case if a situation of panic is created among the husband and wife it will result in creating a chaotic scenario not just for them but also for the people around them and also for the people who are trying to solve the issue.

Case Study 2: Candice
Candice is frail and has dementia. Her dementia has been caused by a history of drug and alcohol abuse.  She lives at home with her niece, Nevaeh, who cares for her. 

Candice receives support at home to shower and receives meals on wheels. Nevaeh is known to be an alcoholic and it is expected that she also takes illegal drugs. 

Lately Candice has been losing a lot of weight. There has also been some unusual bruising on her arms and chest – Nevaeh explained that Candice often falls out of bed, especially if she has been drinking. 

Nevaeh is drunk when you arrive today and you see that she is eating Candice’s meals on wheels.

You also find medications in the kitchen, which have obviously not been given to Candice for some time.  When you ask Nevaeh about why she is eating Candice’s food, she says that there is no point in her giving it to Candice as she just spills it down herself.

When you ask her about Candice’s medication, Nevaeh says that these are old prescriptions that she doesn’t take any more (even though they are the medications listed on the care plan).

She then says, ‘Is it pension day today? Thank god! I’m desperate for some ciggies!’

Albert is dying. As he is enduring his last few days, he asks if he can tell you something. 

You are happy to sit down for a chat, as you often enjoy a chat with him when you visit his home. Albert begins to talk.

You are shocked to hear him confess that, when he was a teacher during the 1960s and 1970s, he committed some sexual offences against girls and boys at the school where he was working. 

Albert cries that he is truly sorry for what he did. Two days later he dies. His family assembles around his bedside and talk about what a gentle and good man he was.

You feel that Albert was genuinely sorry for what he did. You know that you are obligated to report all issues of child abuse, however you wonder if there is any need in this case – it all happened so long ago and Albert was such a nice man.

  1. What action should you take in this situation?
Albert has a secret that he had done something very wrong in his past and he is bearing that alone for Quite some time but before he dies he wants to express his dark secret of life that he does regret and it seems that after that he didn’t commit any such thing ever again this shows that he genuinely regrets what he had done in past He could just keep his secret with him and dies but he choose to tell and he indeed feels happy about it that he shared such burden of his life he is dead now I will keep his secret a secret this is the most appropriate option that I can come up with.

But he indeed has done something wrong no matter what the end was the second option could be is of, to tell the truth, but nothing will change a person is dead and his family only going to miss him creating such scenario is senseless thing to do.

  1. What type of psychological damage may Albert have caused to the school children who are now in their 50s and 60s?
Children who have been sexually abused and assaulted could have developed negative feelings and emotions for the Grown Ups and in most cases, it has been observed that their growth gate hindered by such activity which leaves an impact on their whole life at the time when things happened they may don’t know what exactly happened but in their 50’s and 60s they exactly know what is happened to them and they were providing some necessary information to their young ones go to the people they know so that such things could be avoided or could be reported, etc.
